The State Bureau of Vital Records began recording birth and death records on January 1, 1910, for events occurring in Missouri. Certified copies of most Missouri birth and death records may be obtained through our office. We can access birth records from 1920 to present and death records from 1980 to present.
One primary identification document that includes your name and photo ID such as a driver's license, U.S. military ID card, U.S. passport, work or school ID, etc.
OR
At least two alternate forms of identification (documents must contain your name and the name of the company or organization issuing the document.)
AND
Payment by cash, check or money order made payable to Morgan County Health Center.
1. Walk-in: Same day service is available to walk-in customers at the office. This is the fastest way to obtain a birth/death certificate. You may complete an application and show proper identification with the required fee (please see the fee schedule below).
2. Mail: You may download the application below. Upon completion, it may be sent to our office (along with a self-addressed stamped envelope, photocopy of your drivers' license, and application fee). DO NOT SEND CASH BY MAIL. Certificates will be processed and returned to the sender within 5-7 business days. Information may be mailed to:

How are home births registered?
There is a special form that must be obtained from the Bureau of Vital Records in Jefferson City. The form must be completed correctly, signed by persons with knowledge of the birth, and be notarized before being submitted to the local Registrar in the county in which the birth occurred. It is best to ask for assistance in filling out the form.
When the parents are not married can the fathers' name be placed on the certificate?
The hospitals have forms that must be completed and signed by the father. The completed form must be sent to the State by the hospital with the certificate of birth. To add the name at a later date call 573-751-6378 for assistance.
How can a birth or death record be corrected or changed?
The legal department of the Missouri Department of Health, Bureau of Vital Records must be contacted for assistance.
Where can I obtain marriage or divorce records?
Marriage records may be obtained from the recorder of deeds in the county where the license was issued.
Divorce Decrees may be obtained from the circuit clerk in the county where the divorce decree was granted.
